COMMITMENT TO INCLUSION
REQUIRED
This registers your school with Special Olympics to participate in Unified School programming. If your school currently offers Unified Sports, Inclusive Clubs, etc. with Special Olympics, please check with your school’s Unified Liaison. This form registers your school for all Unified programming that your school will offer.
PARTICIPANT PAPERWORK
REQUIRED FOR ALL STUDENTS
This form is required to be completed for each student. An online version is available for students to conveniently submit without the need to print and scan each form. If your school has jumped on board following the Kickoff event, please have students complete these forms before their first build session.
BUDGET REQUEST
OPTIONAL
This form should be used if your school needs financial assistance. Please note that not all funds asked for will be provided. SOWA will work with each school individually to assess financial needs. Again, if your school has a current Unified program, please talk this over with the liaison, as they might have requests/needs as well. Funds projected for the entire year will need to be included in this document. If you are not sure if your school has additional Unified School opportunities, please contact mlarche@sowa.org.
